COVID-19 Pre-Appointment Screening - Wellness Survey
As part of our protocols, we ask patients to fill out a questionnaire in advance to their office visit. The purpose of that questionnaire is to screen patients for symptoms of COVID-19 in order to reduce the risk of transmission to our staff members and other patients.
